What it changed in my thinking

Accountability and systems thinking are not opposites. A fair response considers intention, context, incentives, tradeoffs, and the conditions under which the decision made sense at the time.

Who it’s useful for

Leaders in safety-critical work, people responsible for incident reviews, and teams trying to build a candid learning culture.

A tension

Avoiding blame must not become avoiding responsibility. A just culture still needs boundaries for reckless behavior and meaningful repair after harm.
