Why I recommend it
I recommend this because it moves the question from who failed to how the system made an action reasonable at the time. That is a more humane and more useful starting point for learning.
What it changed in my thinking
It deepened my skepticism of simple root-cause stories. If an explanation ends with one person’s mistake, we have probably stopped before understanding the conditions that produced it.

Leaders, operators, product and engineering teams, and anyone responsible for incident reviews, safety, or organizational learning.
A tension or limitation
A systems view should not erase individual responsibility. Its value is in understanding behavior within context so accountability can lead to improvement instead of blame alone.
